A US law enforcement official said more than 40 people died, without providing further details.
The truck was found near the railroad tracks in a remote area on the southern outskirts of the city, local broadcaster KSAT reported.
According to councilor Adriana Rocha Garcia, who represents District 4 of San Antonio, another 16 people were taken to hospital.
Three of the migrants found were taken to Metropolitan Methodist Hospital and are in stable condition, according to a spokesperson for Methodist Healthcar.
