Hope: «All vaccinated by the summer». How will this be possible?

“By the end of the summer I realize that all Italians who want it will be vaccinated”. It is a goal and also a reassurance that of the Minister of Health, Roberto Speranza, who spoke about the acceleration of the vaccination campaign in half an hour more on Rai3. It is an optimism that serves the economy, but is also based on numbers that seem to be improving in the future.

The departure was incomplete due to the non-delivery of the doses agreed by some pharmaceutical companies and by the delays of others.

For now in Italy there are 5 million vaccinated. From April onwards, with the approval of the Johnson & Johnson single-dose vaccine, the doses should be sufficient for a mass vaccination. That’s how.

ASTRAZENECA FOR EVERYONE
Between today and tomorrow the circular from the Ministry of Health should arrive with the green light for the administration of the AstraZeneca vaccine even to over 65s, hitherto excluded. Minister Speranza said that “there is new scientific evidence showing that it can be used on all generational groups”.

The vaccination of the school staff, for now 385,000 out of a million have been immunized with the first dose, and by the police. Then it will be up to over 80 and over 70, the category that remained out with the first division of the bands with AstraZeneca only for the under 65s.

There are three vaccines currently available to everyone: Pfizer, Moderna and AstraZeneca. If this can also be given to the elderly (6 out of 10 deaths are over 80), immunization should be faster.

Greater efficacy was observed for AstraZeneca se the second dose is delayed for up to 12 weeks after the first. According to the Oxford University monitoring, the efficacy increased from 59.5% to 82.4% with this time interval. This distancing also has an advantage in expanding the population receiving the first dose of the vaccine.

JOHNSON&JOHNSON
The turning point should come in April with the vaccine monodose of Johnson & Johnson which should lead to simpler logistics and scheduling.

APRIL
In the second quarter of the year, the arrival of 50 million doses, more than half of these should arrive between now and the end of April. With these, the goal is to cover half of the population, always excluding those under 18. By the summer, all those who want it should be vaccinated according to the minister.

SPUTNIK
With these numbers it should not be necessary to resort to the Russian Sputnik vaccine. For the minister, however, there is no prejudice and it can be used, like the others, after the checks and the go-ahead by Ema and Aifa.

SPEED OF ADMINISTRATION
As from the first moment the forces of the regions and the health system have been in place, now those too are needed of the army and civil protection. Nor is the idea of ​​a generalized red zone, excluding Sardinia because it is a white zone, taboo, to concentrate as many forces as possible on vaccination. It would last three weeks and would also serve to reduce the worrying numbers of the contagion.

The hypothesis is 100 vaccinations per day for each workstation. The goal is to reach 6-700 thousand administrations per day, four times the current one. The criterion will be personal and alphabetical, with no other priorities except those already existing and in the process of vaccination and the fragile population. Another aim is to reduce the disparity in distribution between regions. Minister Gelmini said that the government will identify common criteria, “preventing the regions from proceeding in random order in identifying, for example, workers in essential services. Disabled people and their families, caregivers and operators of foster homes must be immediately vaccinated. And it is necessary to accelerate with the teachers, who must be vaccinated in the municipality where they work ».

For cities and larger centers there are spaces available, from sports halls to gyms, through trade fairs and auditoriums (the Cloud in Rome is an example), but also barracks. These are joined by the studies of general practitioners. Smaller centers lack facilities of this kind and so the hypothesis is that the vaccine will arrive almost at home. They should be created mobile units, one for every 20-40,000 inhabitants, creating a vaccination center that goes from municipality to municipality. A similar thing has already been done in Lombardy in Viggiù. An agreement is also being worked on for vaccinations in companies and production sites.

They would be the forces of Civil protection to take action for the so-called last mile, the arrival where the vaccinations are really done, after that the army will have brought the vials to the different regions, with a redistribution of the vials based on the resident population. In addition, there would also be medical specialists for the administration.
