According to the gynecologist and obstetrician Nélio Veiga Júnior, it is always necessary to observe the “maternal causes”, which can cover cases of hypertension, diabetes, autoimmune diseases and some complications that the mother’s own pregnancy presents. Still, from 10 to 20% of cases occur with fetuses that have some genetic disease, that is, that acts chromosomal alteration.
“Also, we always need to check if there has been any kind of infection, which is responsible for 50% of fetal deaths in underdeveloped countries. They can be from toxoplasmosis to rubella. Herpes, syphilis, viruses such as zika and covid, pneumonia, have some bacteria that can colonize this baby and died,” adds Nélio.
In the case of Micheli, who had a fetal death after an advanced pregnancy in about 40 weeks, the higher risk can still be placental insufficiency with conditions of the placenta itself. “In Tati’s case, she was 33 weeks before 37 weeks. This opens a bigger key for us to discuss several causes,” explains the doctor.
It is not common, but it happens
According to research raised by the professional, deaths in more than 20 weeks fell from an average of 33 to 23 per thousand births. Stillbird rates in low -income countries are higher – approximately 36.3 deaths per thousand births. Most cases of intrauterine fetal death occurs in sub -Saharan Africa and southern Asia, 77% of cases around the world.
How is the withdrawal of the baby
Tati and Micheli Machado had to go through the delivery for the fetus withdrawal. The doctor points out that, as directed by the Brazilian Federation of Gynecology and Obstetrics Associations (febrasgo), the priority is knowing how to receive patients.
“We need to explain accurately what happened and guide in the best way. Listening and understanding all the emotions of this patient who goes through a very difficult time. All companions can stay with the patient if this is her desire during withdrawal,” explains the professional.
The indication is that delivery is as natural as possible, that is, vaginally. “There are few cesarean section indications, which is a surgical procedure,” says the obstetrician.
“If she is a woman who already has multiple cesarean sections, if it is, for example, a case of placenta detachment, and if the baby is not well positioned, we can choose the cesarean section method,” he added, indicating that each patient should be evaluated by the obstetric gynecologist himself.
“The labor process directly impacts the way of processing mourning. It is necessary, again, a team that knows how to communicate and that effectively takes care of this family. It needs to validate all the emotion that is involved, because it is really a tragedy,” concludes Nélio Veiga Júnior.
