The Federal Council of Medicine (CFM) published on Tuesday (20) in the Federal Official Gazette new rules and parameters for the bariatric and metabolic surgery in adults and adolescents.
THE Resolution No. 2,429/25 Unifies Standard No. 2,131/2015, which regulates bariatric surgery, together with Resolution No. 2,172/17, which normalizes the same type of procedure for patients with type 2 diabetes.
Among the main changes is related to patients with body mass index (BMI) between 30 and 35, which are eligible to surgery as long as they have type 2 diabetes, cardiovascular disease with target organ injury, early chronic kidney disease due to type 2 diabetes, severe sleep apnea, non -alcoholic fatty disease with fibrosis, transplantation, reflux disorders, reflocation gastroesophageal with surgical indication or severe osteoarthritis.
Another important change in the resolution concerns adolescents eligible for bariatric surgery. Now, patients from 14 years old who have BMI over 40 associated with clinical complications can have surgery. However, the evaluation of multidisciplinary team and the consent of those responsible is still required.
THE CNN the president of the CFM, José Hiran Gallo, explained that the proposed updates in the resolution were made “to comply with a request from Brazilian society”. “The Technical Chamber of the Federal Council of Medicine did a study so that we could go out with a resolution that met the minimum criteria of this morbidity [a obesidade] that affects the Brazilian population, ”he says.
In addition, the president states that a series of recent scientific studies, which underpinned the resolution changes, revealed the benefits of bariatric surgery for adults and adolescents. “This type of surgery will bring a lot of benefit and survival to the patient,” he adds.
Gallo also states that decisions were made after consultations with specialty companies, including the Brazilian Society of Bariatric and Metabolic Surgery (SBCBM).
Why are changes important?
For Alfredo Martins Fontes, head of the Bariatric Surgery Service of the São Vicente de Paulo Hospital (HSVP-RJ), the changes described in CFM resolution are important by “regulating what had already been proven in numerous academic works and clinical practice”.
“From this regulation, patients who can now be categorized in type 1 obesity, ie, between 30 and 35 and who have such comorbidities, may benefit from surgical treatment,” explains Fontes.
Regarding the expansion of access to bariatric surgery for adolescents from 14 years old, the expert states that the update is important because IMC over 40, in young people in this age group, is associated with clinical complications that put them at risk of life. “Now they have some treatment opportunity, something that didn’t happen before,” he says.
For sources, the new resolution expands the possibility of surgical therapy for a large number of patients who fit surgery and, even with scientific proof, had no established regulation. In addition, the standard establishes safety criteria for both the medical and surgical team, and hospitals where the procedure can be performed.
