PF will analyze the profile of Voepass’ crew and safety culture

The Federal Police (PF) must produce two main reports on the Voepass accident in the investigation into the possible causes of the crash. The documents are considered essential for the investigation to determine or rule out the possibility of a crime and to take statements from witnesses.

The first will be a “descriptive” report of the crash site and should be ready within 90 days. The second is the “aeronautical accident” report, considered the main one, and may indicate criminal liability. The latter can be ready within a year.

For this second report, the PF will examine the same material as Cenipa (Aeronautical Accident Investigation and Prevention Center), but the conclusions are reached independently.

At this stage of the investigation, the PF will cross-reference various pieces of information produced by the experts, including the crew profile and the company’s safety culture.

The report on “aircraft accident” is a document similar to the Cenipa report. The difference is that the PF investigates or rules out the possibility of crimes.

For this analysis, federal criminal experts monitor all examinations, trials and tests carried out by Cenipa, including the sharing of information about the wreckage, engine and black boxes of the aircraft.

The PF’s conclusions should provide an overview of the accident. See the steps that should be included in the report:

  • accident overview;
  • detailed description of the accident with chronology and meteorological data;
  • analysis of human facts with profile of those involved, qualifications and health status;
  • evaluation of procedures followed and training received by the crew;
  • analysis of material factors with aircraft condition, results of inspections and maintenance;
  • analysis of operational procedures with procedures followed and whether there was adherence or any deviation;
  • analysis of communications;
  • organizational aspects with examination of company policies and procedures and safety culture;
  • environmental factors on site conditions and relevance to the event
  • external interference that may have influenced the accident

Other preliminary and complementary reports may also guide decisions in the PF investigations. A report describing the preliminary location should be ready this week.

The descriptive report does not analyze the causes of the accident. This document must include information about the positioning of the aircraft on the ground where it crashed, the number of victims and the external damage in order to understand the context of the accident.
